Intake Process [Student]
To help us craft an individualized coaching plan and match you with an ideal coach, please complete all four steps on this page:

Please be sure to click each “submit” button individually. Otherwise, your answers may not be saved or submitted. We recommend completing these forms using a computer rather than a mobile device. If you have difficulty using this webpage, we can also email each form individually. All data you provide is safely stored and kept strictly confidential. Relevant information will only be shared with your coach and CTL Directors.

Step 1: General Info Survey
This survey (5-10 minutes) asks for some contact and personal information as well as your primary reasons for coaching, any specific requests you may have, and your general scheduling preferences.
Step 2: Executive Function Survey
For students under the age of 14: we recommend that parents complete this survey on behalf of or in collaboration with their child.
For students over the age of 14: we recommend that the student first complete this survey either independently or in collaboration with parents. Parents are also welcome to submit a second survey on behalf of their child but this is not required or expected.
Please note: results will be emailed to you automatically. We will review these results in more detail during the intake session.
Step 3: Billing Policies
This form (~5 minutes) details our billing rates and policies, including the fee for the intake process. It will also ask you to provide credit card information which is encrypted and stored securely using Stripe. For verification, there will be a $25 holding fee placed on your card that will then be applied toward your intake fee or first session.
Step 4: Schedule Intake Session
If you are scheduling a video intake session, please use the calendar below. You will receive automatic email and text reminders through the service Calendly; you may have to check your email spam box.
If you live in New York City and would prefer an in-person intake session, please skip this calendar and instead email Rebecca@CTLtutors.com to coordinate a meeting.
There is a 24-hour cancellation policy for all scheduled intake sessions; late cancellations may result in a fee.
For students under the age of 14: depending on the student’s age, this will either be a parent meeting or a family meeting; at least one parent/guardian must attend.
For students over the age of 14: this will be an independent meeting with just the student. After the student intake session, we will follow up directly with a parent to schedule a separate meeting.
